Cerebral vasomotor reactivity in micro- and macro-vasculature of patients with severe steno-occlusive internal carotid artery lesions

Abstract

The cerebral-vasomotor-reactivity (CVR) is evaluated in 27 patients with severe carotid artery steno-occlusive lesions in both micro- and macro-vasculature. We have combined two diffuse optics modalities: diffuse-correlation-spectroscopy (DCS) to measure local relative-cerebral-blood-flow (rCBF) and near-infrared-spectroscopy (NIRS) to measure oxy-hemoglobin (HbO2) and deoxy-hemoglobin (Hb) concentration changes. In addition changes in cerebral-blood-flow-velocity (CBFV), macro-vasculature, is followed by transcranial Doppler (TCD). Upon acetazolamide (ACZ) bolus, we have observed a significant increase in HbO2, rCBF and relative-cerebral-blood-flow-velocity (rCBFV) and a significant decrease in Hb. In healthy subjects, a significant agreement (p=0.33) between micro- and macro-vascular CVR measures was observed which did not hold true in the case of patients with severe steno-occlusive lesions (p=0.02).
